Problem:  Lori  Geist at San Bernardino asked about the User Reports including the User Summary.  At the end of the User Summary is the User County by Last File Date.  Shown below is a User Summary for C36 Jan. 1, 2002 – Jan. 31, 2002.  It shows Eugene Jenkins with 25 and a User 0 with 4.  There is no user 0.  Eugene has not been doing this for 3 years.  His ID is deactivated and has been for at least a year.  He is User ID # 1.  This problem came up last year at San Diego with User ID #1 who has never done anything in the system as having a count also.  This makes all of this User Summary information questionable.  This needs to be fixed.

Problem:  Memorial Hospital, H617, in Stanislaus County has gone from being a stand-alone hospital to now as of today dialing into the county.  They had a verification letter that was not standard.  They would like to use that with their special stationary.  The other hospitals are using the standard Verification Letter; so to change the Verification Letter for Memorial would change it for all the hospitals.  Can the Verification Letter be individualized for different hospitals when more than one dials into a county.

Problem:  In the new AVSS 4.11 release, the LFN ASSIGNMENT under the DEATH

 CERTIFICATE option assigns the local file numbers without first printing a test certificate. I understand why it was considered unnecessary compared to the birth as no bar code is printed on the certificate. However, in the past, we have experienced residual data in the print buffer, which often times will come out on the test certificate. Recommended Solution: Reincorporate the test certificate into the LFN ASSIGNMENT under the DEATH CERTIFICATE option.

Problem:  5/16/02 When researching the birth certificate for Viviana Rodriguez (BPF 20795605) at Northridge Hosp. (#247) that was identified by Vital Records as a problem with the List to be sent to the Health Department the following was found: 1. The record was not a problem after all. The correct procedure was followed. 2. It was put on the list 5/15/02. What was interesting was the record still showed up in “Old List to be Sent to the Health Department” but not in “List to be Sent to Health Department” after it had been put on the list. I don’t know if this is an indication of anything but thought I’d share with you just in case.

Problem: Certain indexes in the FSX global have meaningful data stored as the node value in addition to the index values.  Currently, there is not an easy way to access this data in the Report Generator.  Some examples of these type of indexes are:

Form=all, index=%UPDT, value=id information
Form=CMR, index=ICD, value=SentDate
Form=CDC, index=BD, value=LFN of linked birth certificate

Solution: Add a new system variable %V0 to the Report Generator to allow access to the FSX index node value.

Problem:  An error has been found at OVR: In certain cases the ID codes in the %USAGE info on birth certificates isn’t being recoded to have a % (percent sign) which causes user reports to mis-report that state users are accessing records when they are not.  Line USAGE^FSTRNOSR quits when the first entry in a usage list has a % character. It doesn’t account for there being subsequent entries that do not have the %.

Solution:  USAGE^FSTRNOSR should be commented out. (USAGE+2 is okay.) This will have to be implemented at all LRD sites.

Problem: Regarding the revised 1994 Death Certificate.  AVSS please update field 11 in AVSS on death certificates, there is no “11A or 11B”.  There is just field 11 with 3 boxes to either enter a “YES”, “NO”, or “UNK”.  On a couple of occasions I have needed to amend a death certificate regarding field 11 to change it from a “NO” to a “YES” and it’s impossible.

Problem:   Foreign import records from Kaiser South are arriving with values for MAR and DECP when MAR=YES. In AVSS if MAR=YES we don’t prompt for DECP leaving it undefined. The edit check for father’s name items (6A, 6B and 6C) assumes that if DECP is defined MAR=NO. This check will then disallow edits (or amendments) to 6A B or C even though MAR=YES.

Solution:   Modify the edit sub field check for HCA, 6A to account for the case that MAR=YES and DECP is defined.

Currently, with this feature you can only select a record by name. You have to scroll through the list until you find the desired record. This could also eliminate the desire to use the “Old list to be sent to the Health Department” feature, which allows record selection by file number.

Problem:  Duplicate hospital records are being transferred to LRDs. 

Solution:   The only time a record would get into ^FSTRAN at the hospital to be transferred to the LRD is when it is put on the LIST TO BE SENT TO THE HEALTH DEPARTMENT.  That way it would cut down on duplicates.

Problem:  Sue called yesterday about a cmr file nbr missing when she did her Missing File Number Report.  She said that no cmrs were missing so it wasn’t scratched.  I tested this in PCAD using C01.  I registered cmrs sometimes letting it time out to see what would happen.  I finally got the right results.  I started to register a cmr, but I only went through the disease and let it time out.  This is the response I got:

It does not file the cmr, but an ID is assigned and when another cmr is registered that number is not used even though a cmr was not filed.  The next cmr ID number is used.  Then when you do a Missing File Number Report 12 is listed as missing even though there was never a cmr to go with it.  She can’t go back and edit a cmr later to reuse that ID nbr:
