Problem/Suggestion: Transitioning LHDs to use of DMR could require considerable training, habit change, etc. (and re-training when staff changes), and the choice of "DMR" ("pending") vs. "CMR" could be confusing.
Solution: Could AVSS be programmed to print out a line listing of remaining DMRs on a weekly basis as a sort of "tickler" for county staff that "Pending CMRs" are still awaiting some action/disposition?
This could be a simple option after the user prints out the WMPR. (e.g., "There are XX pending CMRs. Do you want a printed copy of pending CMRs?")

Problem/Suggestion: Some reallocated records have bad census tract data. The LRD that registered the document has entered "dummy" census tract codes in the Census Tract fields. (They may arbitrarily assign a certain code to all Alameda County resident records to make their analysis easier.) We then have to allocate staff time to identify and clean up the "garbage" census tracts and replace them with real census tract data. This could be remedied by two methods: either don't transmit census tract data with the reallocates, or don't allow LRDs to enter unreal census tract data.

Problem/Suggestion: Many times, when counties call OVR with edits, they need to have the edits updated on their system immediately. Currently there is no way to do that. Also, when we haven't connected to a county for reallocates, we have no way of testing the system to see if we are running into a time job problem, or a logon problem. The way we test it for receptions is to do a daytime startup, to see if we connect.
Suggestion: An option for a Daytime Reallocate Startup be added to the OVR system.

Problem/Suggestion: Currently the Mother's Last Name (MLN) is a field that is not linked between the Delivery Record and the Hospital Birth Record. They should be. The Date of Birth for the Mother of Child is linked which is great and if the MLN were linked it would save much time and effort especially when the MLN is particularly long and difficult to spell. The person entering the Delivery Record already has to type the MLN so linkage with the Birth Record would mean information already typed in would be displayed. This would save time and increase the accuracy of the data.

Problem/Suggestion: Some CMR cases that get marked as a non-disease after a follow up review of the data. Peter Chen proposes adding a special DISEASE designation to denote these cases, e.g. DELETE. The CMR clerk would then just edit that field and change the disease value to DELETE. This has the benefit of retaining the record for reporting purposes instead of scratching it.

TAG Recommendation: OK to add the choice of ~DELETE to the DISEASE list in the CMR form. The tilde (~) prefix will force this choice to the bottom of the list in ASCII sorting sequences. CMR reports will need to be modified to discount such cases. The original ICD code will be retained so that the original disease choice will be preserved. It was decided that a new CMR menu option called (for example) MARK RECORD FOR DELETION would reset the value to ~DELETE automatically. Gail Cayton suggested that marking such cases at the state level should result in the records being automatically reallocated to the LRD (see AMR 00-037). After some discussion this concept was approved. It was agreed that Sacramento County should be the beta test site

Problem/Suggestion: CDC Weekly Mortality Report - Record Selection criteria:
Currently, Los Angeles is part of the 122 Cities Influenza-Pneumonia Surveillance System. In the past, selection of death records using underlying cause of death was sufficient to be included in the surveillance report. However, case selection criteria have now changed.
In order to be included into the tables for death by age, the use of the term "any mention of" has been incorporated into the case selection algorithm. Any mention of Influenza or Pneumonia, regardless of where it appears on the death record, is sufficient to count.
We do not code any mention of Influenza or Pneumonia. In addition, three other conditions are of public health importance, and could be useful in certain studies. These include Cancer, HIV, and Diabetes.
Solution: Add a new variable to the Death Record called AMO (any mention of). The only values that would be accepted would be I, P, C(cancer), H (HIV), or D (diabetes) and any combination of the five letters. The max length for the field would be five (5). Single letter values, as well as all combinations, would be acceptable responses.
For Example: Any Mention of Influenza would be coded as I. Any Mention of Pneumonia would be P. Any mention of both would be IP. Any mention of Cancer would be C. All five would be coded as IPCHD. Any mention of any combination of two, three, or four would also be valid.

TAG Recommendation: It was suggested that the need to accommodate one-letter codes in order to report 'Any Mention Of' could be accommodated by using 107A (Cause of Death). There was no consensus on adding another prompted field to the CDC record. After some discussion it was agreed that a new field called AMO would be added to the CDC, but not prompted. This would allow LRDs who wish to add this field to do so by using the EDIT CERTIFICATE suboption while not imposing an extra prompt on other LRDs.

Problem/Suggestion: With the advent of printing of the registration date (Field 17) on the birth certificate during the LFN ASSIGNMENT procedure in Ver 4.9, and with the change of the birth certificate forms on 1/1/00, serveral LRDs have had problems with overprinting the registration date on the instructions for Field 17. This has resulted in rejected certificates and the unfortunate necessity of producing replacement copies. Part of this outcome is due to the closer tolerances that are required when Field 17 is printed in addition to the LFN at the right-hand top corner of the form and its corresponding bar code at the lower right-hand corner. Additionally, there may be variations in the top margin on the blank birth certificate forms, and there is very little space available to print the bar code in the bottom margin since OVR has instructed hospitals to detach the privacy notification portion of the form before forwarding the certificates to the LRD.
Solution: As suggested by Peter Chen, the relative position of the bar code with respect to Field 17 could be optimized since its original position did not account for the printing of Field 17. Second, rather than having a fixed vertical offset, allow the user to change it during the LFN test print as shown below:

Problem: While AVSS/CMR has performed well throughout the state and
provide a standardized method of collecting, communicating, and reporting
CMR data at the state and local levels, there presently exist non-AVSS
systems that also collect CMR data. There is evidence that there may be
more of these "foreign" systems in the future, thus leaving California
without a single, integrated system for communicable disease reporting.

Solution: Develop a capability for AVSS to import CMR data from other
systems using the variable format approach that is used to import birth
record data from Southern California Kaiser hospitals. This ASCII format is:

The advantage of this format is that not all variables have to be reported,
only those that AVSS requires. Thus, different systems could report fewer
variables than exist in AVSS, provided that the minimum data requirements
were met. Also, there is no need to design a complex record file layout that
might change in the future. AVSS/CMR variable names and definitions are
listed below; an asterisk marks those variables that would be required by AVSS
from a foreign system. If any of these variables are missing or do not pass
AVSS edits, they would be marked as ^SKIPPED and result in an incomplete
record.
These variables would be imported into an FMR form and
filed by FMRID at the County of Occurrence (XIND) level.
Later, an AVSS suboption under CMR FOREIGN INPUT would allow
the user to move all accumulated FMRs into the CMR form, i.e., there would be
no human interaction required to transform an FMR into a CMR as there is for
linking a HCA to an LCA. Incomplete FMR records would not be transformed.
Also, there would be little if any control over the quality of imported data;
for example, once the FMRs were converted into CMRs, a duplicate FMR could
be imported. The CMR records would be cross-indexed by FMRID.

Problem: Duplicate HCA/LCAs are occurring as the result of the following sequence:
1. A stand-alone hospital creates an HCA and it is transmitted that evening to the LRD.
2. Later, the hospital applies LIST TO BE SENT TO HEALTH DEPARTMENT and sends the paper copy to the LRD.
3. There are problems with the hospital-to-LRD communications so that the revised HCA containing the SENT node does not get transmitted to the LRD.
4. The paper arrives at the LRD and the LRD links to the HCA.
During registration, the LRD will see the following warning: WARNING: HOSPITAL CERTICATE HAS NOT BEEN FLAGGED AS SENT.

Problem/Suggestion: A user at Los Alamitos Hospital (H391) related a problem with ^FILE at the new race prompts (18ABC and 21ABC). Whenever the record was saved at one of these fields, the save point would be lost and the certificate would be considered complete although the remaining half is blank. After further investigation, the problem was found to be in FSHCASF1. About 4 years ago, a special check was added to disallow filing at non-standard BC fields [96085]. This is to prevent UNDEF errors at the LRD site when trying to complete a certificate at an invalid start point. The current problem is that the new race fields were not added to this list. I don't believe the problem is severe enough to warrant a full hospital distribution fix, but we could update specific sites on a per request basis.
Solution: 1. Add 18ABC and 21ABC to this special ^FILE list
2. Remove 18, I18, 21, I21 from the ^FILE list
3. Update the FSHCASF1 routine to search the entire field list for the next incomplete field if the ^FILE field is not found

If you go to the Signature stamp, it would work even better to have the signature stamp and date of registration pass on the day they are received. Then do a batch with the LFN numbers.

Problem/Suggestion: This request relates to the DMR (Pending Files System). When you go to System Option, Generate Report and select Alpha. You will find Name and File #. Here in San Diego, I print the report up after cases have been added. Therefore, name and file# are not enough information when printed up. We recommend that you add Diesease, Date Rec'd, and Comment in the Alpha Listing.
After the request when printed up we will see the Date Rec'd, Name, Disease, Comment, which will allow us to know information regarding the pending case. When cases such as Meningococcal, Malaria... arrived off the FAX immediately they are placed in the DMR and assigned to person on call for that day. This eliminates duplicate cases. Presently looking at the list does not provide enough information. And nurses will ask whom or do we have the case.

3. When OVR is entering the OOS BC, OVR assigns the 98xxxx sequential number. When the write out occurs, the program looks for "9898" in positions 6-9, writes these records out, and converts positions 6 and 7 to "00". Example - AVSS Record LRN 1199998980001 would
be converted to LRN 1199900980001.

Problem/Suggestion: Accuracy of Address Data on Birth/Death Files.
I believe that the accuracy of address data would be greatly improved if those entering the data would not be allowed to type in the zip code and allow the city to pop up automatically. It has been my experience that city data is much more accurate than zip code data; patients simply do not seem to know their zip codes. And those entering the data are systematically not careful about double-checking the city. Thus we spend a lot of time trying to match the street address with the correct city when census tracting, and oftentimes can't figure out the correct city. Also many certificates are allocated to us that don't belong because they have the wrong city stamped on them. We then have to contact the state to verify the residence in the case of deaths. With births, addresses cannot be verified

Evaluation: At present, the entry of 5-digit zip code will automatically default three fields: the residence state, county, and city, but these defaults can be easily overwritten by the user. Also, the entry of a less-than-5 digit zip code will present a pick list of potential matching cities. The city-zip code correspondence table in AVSS is kept current by the AMR process. Reprogramming the data entry sequence would impact a large number of users and require their retraining.

Problem/Suggestion: Peter reported that he received a call from Pasadena from someone who
was having trouble entering a home birth. Here is the scenario:
register at hospital birth record specify HOME (863) for HOSP prompt (site code) do data entry until reach item 5A for item 5A enter HOME get an error message saying that you can't change the value of the site code...
This occurs because the 5A list location is set to be ^FSL("HOSP","CA",^T($J,"I5D"),
and I5D for a city LRD is the surrounding county, 19 in this case. So the user is using the LA county list of hospitals rather than the city LRD list which triggers the error condition in the 5A,CHK,0 subfield check.
We though it was odd that we hadn't heard about this before so I dialed into Berkeley and did some queries on the values of I5A. There were fourteen 801 codes and one 861 code. For the 861 code the data entry person had put in OWN RESIDENCE. I called the data entry clerk to see how they did the data entry that resulted in I5A having an illegal 801 code saved. It turns out that they proceed as described above and then after they get the error message they put in '--' when re-prompted for
5A. The value is verified but it doesn't change the value from their previous list lookup and they end up with 5A=HOME and I5A=801.
The solution to this problem is to use a different variable in the list location for item 5A. We need to save the value for CNTY when we are resolving the site code prior to data entry. This means two modifications: add a PRE check to item 5A to initialize ^T($J,"CNTY") if it is undefined (a
previously entered record, we can use the value of XIND to determine the proper value of CNTY), and use ^T($J,"CNTY") in the search location for item 5A: ^FSL("CNTY","CA",^T($J,"CNTY"), instead of ^T($J,"I5D").
There is also a side effect from using OWN RESIDENCE above. We only look at the values for 5A beginning with HOME and RESIDENCE for coding Box A to 0000. OWN HOME doesn't meet this criteria and so we end up with Box A = 0998 (This is programming in ^FSF("HCA","5A","CHK",1).)

Problem/Suggestion: I got a call from Alameda that a particular death record had not been reallocated. Upon study I discovered that when we have a reallocate record from a city LRD going out of state (residence not CA) the record is not being reallocated to the surrounding county LRD.
Solution: move the line which causes the reallocation to the appropriate county LRD to before the test for out-of-state residence.
Corollary: This same problem exists in the birth certificates. We will need to run some reports at the city LRDs looking for this condition and reallocating them to the appropriate county.

Problem/Suggestion: From time to time we get a <MAXSTR> communications error. This happens primarily on the CDC certificate when one of the extremely long text fields is edited. The %USAGE,N node contains the id and time stamp of the user making the change as well as the old and new values. When the text strings get more than 100 chars we can sometimes get errors during data entry or during communications when we modify an existing entry's id to add the % to indicate that the user is not a
user on the receiving computer. So my suggestion is to put the old and new values in cases where the %USAGE,N node gets too long into sub-level entries:

Problem: AMR 98-123 was implemented to automatically set Field 15A on the LCA when it was absent and a successful birth-death match occurred at the local level, but I15A was not set. This has resulted in several problems:
1. Some reports use I15A and thus there is a difference between the LCA display and report listings.
2. Matching at the state level has resulted in some false matches and thus 15A has been inappropriately set.
3. There is a difference between what is on the paper and what is on the LCA. Note that the new variables 15AD and I15AD are being set for birth-death matches.

1. Disable the automatic setting of 15A upon a successful birth-death match.
2. Run a 'zap' program to reset 15A to null or undefined when I15A is missing.

Problem: Users frequently (erroneously) enter a date of birth (Field 4) equal to the date of death (Field 7) or make other errors related to these two important dates on the CDC. This results in an adult decedent being misclassified as an infant death.
Solution: Although there are no edits for the CDC (since LRDs are using it to produce a database from pre-registered death certificates), it is appropriate to introduce user validations for the date of birth (DOB), date of death (DOD), and age fields as follows:
1. If the age of death as computed from Field 4 (DOB) and Field 7 (DOD) indicates an infant death and Field 5 (Age) and its components do not, then require the user to edit these three fields until the error condition is corrected.
2. If the age of death as computed from Fields 4 and 7 is greater than 365 days but Field 5 and its components indicate an infant death, then require the user to edit these three fields until the error condition is corrected.
3. If Field 4 is greater than Field 7, then require the user to edit these two fields until the error condition is corrected.

Problem: The CMR form REPORTER variable has evolved from its original intent, which was to record Reporter Type. In fact the prompt text remains as REPORTER TYPE. It is now composed of two components: the user pick list value or reporter and a computed variable, RPTRTYPE, which has the values of: PUB, PRV, MIL, or BB. Since RPTRTYPE is not contained in the variable list for the CMR/DMR/FMR forms, it may not be obvious that it is possible to run Query Database reports on it.
Solution: Change the prompt text of REPORTER from REPORTER TYPE to simply REPORTER. Add RPTRTYPE to the CMR/DMR/FMR forms with the prompt text (though not prompted) of REPORTER TYPE to aid in running QUERY DATABASE reports.

Problem: When an LFN edit is performed at OVR the existing ^FSDSB entry will prevent the second LCA with the same LFN from being written to DSB, so that the corrected record will be in AVSS, but not in BID.
Solution: Create a new OVR-only procedure called: REMOVE ENTRY FROM FSDSB GLOBAL that will allow OVR staff to selectively remove ^FSDSB entries whenever an LFN is edited.

Problem: As the number of AVSS users has grown there is an increasing chance of a security infractions since there are presently no controls on its passwords, other than expiring after a specified time. For example, without User Name level of security, there is a likelihood that a simple password will be discovered when a second user enters a password that has already been used.
Solution: Place more restrictions on the type of passwords that can be used, including:
1. Disallow passwords composed only of numbers or only of alphabetical letters.
2. Require that non-alpha characters be embedded in the password, not as the first or last character.
3. Require that the password be a minimum number of characters, say 7 or 8.
4. When (if) AVSS requires a User Name, disallow the User Name as the password.

Colombian and Moroccan are misspelled on the race list.
Solution: Change the California codes and the NCHS reporting codes for the 19 races from Other (Specified) to Other (Unspecified/Unknown). Correct the spelling of Colombian and Moroccan.
