Problem:  Lori  Geist at San Bernardino asked about the User Reports including the User Summary.  At the end of the User Summary is the User County by Last File Date.  Shown below is a User Summary for C36 Jan. 1, 2002 – Jan. 31, 2002.  It shows Eugene Jenkins with 25 and a User 0 with 4.  There is no user 0.  Eugene has not been doing this for 3 years.  His ID is deactivated and has been for at least a year.  He is User ID # 1.  This problem came up last year at San Diego with User ID #1 who has never done anything in the system as having a count also.  This makes all of this User Summary information questionable.  This needs to be fixed.

Problem:  Memorial Hospital, H617, in Stanislaus County has gone from being a stand-alone hospital to now as of today dialing into the county.  They had a verification letter that was not standard.  They would like to use that with their special stationary.  The other hospitals are using the standard Verification Letter; so to change the Verification Letter for Memorial would change it for all the hospitals.  Can the Verification Letter be individualized for different hospitals when more than one dials into a county.

Problem:  In the new AVSS 4.11 release, the LFN ASSIGNMENT under the DEATH

 CERTIFICATE option assigns the local file numbers without first printing a test certificate. I understand why it was considered unnecessary compared to the birth as no bar code is printed on the certificate. However, in the past, we have experienced residual data in the print buffer, which often times will come out on the test certificate. Recommended Solution: Reincorporate the test certificate into the LFN ASSIGNMENT under the DEATH CERTIFICATE option.

Problem:  5/16/02 When researching the birth certificate for Viviana Rodriguez (BPF 20795605) at Northridge Hosp. (#247) that was identified by Vital Records as a problem with the List to be sent to the Health Department the following was found: 1. The record was not a problem after all. The correct procedure was followed. 2. It was put on the list 5/15/02. What was interesting was the record still showed up in “Old List to be Sent to the Health Department” but not in “List to be Sent to Health Department” after it had been put on the list. I don’t know if this is an indication of anything but thought I’d share with you just in case.

Problem: Certain indexes in the FSX global have meaningful data stored as the node value in addition to the index values.  Currently, there is not an easy way to access this data in the Report Generator.  Some examples of these type of indexes are:

Form=CDC, index=BD, value=LFN of linked birth certificate

Problem:  An error has been found at OVR: In certain cases the ID codes in the %USAGE info on birth certificates isn’t being recoded to have a % (percent sign) which causes user reports to mis-report that state users are accessing records when they are not.  Line USAGE^FSTRNOSR quits when the first entry in a usage list has a % character. It doesn’t account for there being subsequent entries that do not have the %.

Solution:  USAGE^FSTRNOSR should be commented out. (USAGE+2 is okay.) This will have to be implemented at all LRD sites.

Problem: Regarding the revised 1994 Death Certificate.  AVSS please update field 11 in AVSS on death certificates, there is no “11A or 11B”.  There is just field 11 with 3 boxes to either enter a “YES”, “NO”, or “UNK”.  On a couple of occasions I have needed to amend a death certificate regarding field 11 to change it from a “NO” to a “YES” and it’s impossible.

Problem:   Foreign import records from Kaiser South are arriving with values for MAR and DECP when MAR=YES. In AVSS if MAR=YES we don’t prompt for DECP leaving it undefined. The edit check for father’s name items (6A, 6B and 6C) assumes that if DECP is defined MAR=NO. This check will then disallow edits (or amendments) to 6A B or C even though MAR=YES.

Solution:   Modify the edit sub field check for HCA, 6A to account for the case that MAR=YES and DECP is defined.

Problem:  Sue called yesterday about a cmr file nbr missing when she did her Missing File Number Report.  She said that no cmrs were missing so it wasn’t scratched.  I tested this in PCAD using C01.  I registered cmrs sometimes letting it time out to see what would happen.  I finally got the right results.  I started to register a cmr, but I only went through the disease and let it time out.  This is the response I got:

It does not file the cmr, but an ID is assigned and when another cmr is registered that number is not used even though a cmr was not filed.  The next cmr ID number is used.  Then when you do a Missing File Number Report 12 is listed as missing even though there was never a cmr to go with it.  She can’t go back and edit a cmr later to reuse that ID nbr:

Problem:  In the past Central Valley (HCA111) also could create HCA’s for associated (taken over) hospital Hanford (HCA109). However in AVSS 4.11 the list entry for Hanford was removed. I could find no AMR requesting the removal. Talked to PMC and he didn’t know anything about the removal either. Problem:  Trisch called to say that Donna Olivieri of St. Joseph Hospital, HCA075, had entered her password but AVSS said it was incorrect.  She entered another password, which AVSS took and said her password had expired even though she had only changed it 3 weeks ago.  She went in and registered a hospital birth record, but when she was finished she noticed it said General Hospital not St. Joseph.  She went out of AVSS and disconnected and came back in and used her St. Joseph password and got in and registered the birth at the correct hospital.  She called Trisch, and Trisch called us.  I scratched the General Hospital record.  I checked the ID File: User Last Login.  Several things didn’t look right, several people were listed in the User Last Login and when I tried to Deactivate the ID, I couldn’t.  The name would not appear, but it would say ‘is this the person’.  If I said yes, it said the ID was deactivated, but the name had appeared on the list of active users.  I told John and he checked and found at least 5 people with multiple IDs.  Someone had created new IDs for people instead of editing the ID.

Solution:  Do we need to put code into AVSS ID file (add/edit) to prevent users from duplicating a name. What happens is the latest name replaces the earlier one and then the earlier one no longer has a ^FSI(“NAME” associated with it.

Solution:   The solution is to have an IRACE1 item with the code value from the selection RACE1. In addition to solving the problem of losing the race value this will also allow us to prevent the problem you described in your 4th paragraph (“If ethnicity was first entered erroneously...”) because we can reset IRACE from IRACE1.  Add the IRACE1 item to the CMR.  Also modify RACE and IRACE coding to correct the problem when ethnicity is changed from YES to NO.  Since info has been lost we cannot reliably create an IRACE1 item on previously entered records. So IRACE1 will only exist on CMR cards created after the site has been updated.   In Ron’s new CMR file layout we should use IRACE1 instead of IRACE in columns 334-335 if it exists. Otherwise we will use IRACE.

When there is more than one segment match for an address, we cannot assign census tracts and blocks.  For instance, address 370 Juana Ave yields two matching segments:

The interactive census tracing utility doesn’t allow you to choose a segment.  This is most often a problem when a street has two possible different prefixes as in the example above.  In the past, we used the function CT= to force the census tract; but now that we are assigning blocks and longitude/latitude, forcing the census tract no longer works.

Problem: We lose the last good value of ^FSU(“DATE”) when there is a ^FSU(“FSDBBAD”)=2000 error (i.e. the current date is less than the last good date.)

Solution: In FSPCTIM don’t set ^FSU(“DATE”) if a 2000 error is generated. If we have the last good date we would have the capability of having the user fix this problem themselves:

1. AVSS should tell the user what the problem is (in the FS routine) and that they need to shutdown and reset the date (e.g. Your computer currently is set to a date in the past mm/dd/yy. Please shutdown normally, say N to the backup prompts and reset the date and time by typing DATE and enter and then TIME and enter.)

2. The time job checks: if there was a ‘2000’ error but the current date compares properly ( ‘< ) with the last good date remove the ‘2000’ error condition.

Would it be possible for AVSS to generate a canned  report that lists facility names and addresses for the different systems?  We are having trouble keeping track of the name and address changes, because we have different users requesting changes.  Then those using the data are not aware, for instance, if at a particular site a hospital changed into a retirement home.  The lists posted on the AVSS website are often outdated.

Problem/Suggestion:  The AVSS Net system UPS is connected to a Unix server that can send initiate a telnet session on AVSS. I propose we create a hidden option (to be named later) in the ZSE global, which starts a routine in the MGR UCI. This routine will set ^ZSE to a special value “M SHUTDOWN” and create a file named “AVSSD.CTL” in the AVSS directory. FSPCTIM will be modified to check the value of ^ZSE. If it finds the special value “M SHUTDOWN” it will reset ^ZSE to null and initiate a shutdown. The A.BAT file will be modified to not chain to the backup batch file if the AVSSD.CTL file exists. We could also modify the beginning of the A.BAT to test for this file and prompt for its removal.  The power fail routine (ZPWRFAIL) now accounts for multiple starts (ignores if already rec’d) and also broadcasts a warning message to the users. It hangs for 30 seconds before setting the flag for the time job to initiate a shutdown.
