V1LO is the external date the user specifies, %V1LO is the internal version of V1LO-1 so that when we start stepping thru the index we start on the correct day. I should be passing V1LO and %V1LO+1. The example attached to the AMR would be affected: 1/27/1997 is Day 2 of week 5.

Problem/Suggestion: The National Center for Health Statistics is requesting that the fetal coding be changed to a new format. Enclosed is a copy of the package received detailing procedures and other information necessary to perform the conversion.
NCHS is also requesting that data be submitted in both new and existing formats until further notice. Please modify AVSS accordingly. Generating new NCHS fetal tape to file is requested.
Please prioritize this conversion, and allow sufficient time for testing. The State of Connecticut plans to start testing the conversion in January 1997. NCHS is expecting both files during the first quarter of 1997.

Problem/Suggestion: The current AVSS system does not allow data entry of morbidity reports once they are received. Data entry must wait until the case history is complete (for Kawasaki Disease, this can take up to 3 months). As a result, we have no automated internal tracking system FOR REPORTED INFECTIOUS DISEASE. Due to the nature of infectious disease, and with over 100 reports received each week, this seriously impairs our ability to access data in a timely fashion (e.g. how many cases of E. Coli 0157:H7 were reported during the past week when an outbreak is suspected). It also impairs our ability to identify duplicate/triplicate morbidity reporting, and to identify the status of suspect/pending cases and the case worker. We propose adding a "case status" variable which would allow suspect and/or pending cases to enter the system, while only true cases are reported to the State. We also propose a "comments" variable be added to readily identify staff members assigned to pending and suspect cases.

Problem/Suggestion: A law may be passed which will require us to collect the social security numbers of the parents. Because of the uncertainty of if and when this law will be passed, we would like you to program the new AVSS system to be able to prompt the hospital clerk for the mother's and father's social security number effective July 1, 1996. We do not want it activated, however, until we tell you. This way if and when the law is passed, AVSS will be ready. At that time, you would also have to fix the tape1 output program so we can bring the social security numbers of the parents over to our system.
The above scenario presumes that we will receive the new AVSS system for testing by May 15, 1996, If we do not have 2-3 weeks for testing and improvements, it will not be possible to install the new system by July 1, 1996, and this will have to be made on the current system.

Problem/Suggestion: We received the enclosed memo from the Social Security Administration informing us they were making changes in the Enumeration At Birth (EAB) record layout. According to Shirley Linton, the contact person mentioned in the memo, the actual starting date to produce files using the new record layout has not been determined, but it would be on or about August 1, 1997.
Also enclosed are both the old and new record layout requirements. Please create a new program to produce the Enumeration At Birth File. Both the old and the new programs should remain on AVSS. The new program should be created as soon as possible so that we can test it, and it will be ready to use when the Social Security Administration announces the starting date.

Problem/Suggestion: We recently noticed an error being made in the coding of a "home" birth or a "born on arrival" birth entered into AVSS. The place of birth (3C), is being coded to "99" instead of being left blank. Whenever a "home" birth or a "born on arrival" birth occurs, the record is entered into AVSS, and then County of Birth (3A), City/Town of Birth (3B), and Place of Birth (3C) are edited. When the Place of Birth is not one of the maternity hospitals, it should be coded to blank.

Problem/Suggestion: Communicable disease reporting is often duplicated (and even triplicated) due to the various providers a patient sees throughout the course of his/her illness. To determine of a client has been reported, we look in the AVSS system, which currently displays only name and file number as unique identifiers for each patient record. However, with over 300 patients with the last name "Nguyen" and 10 of those with the same first name, the currently displayed variables are insufficient to determine unique client records. We propose that the patient's date of birth and gender also be listed to save valuable time looking through individual records in display mode to determine if the case has been reported.

If the county (of residence of mother) is code "999" (unknown nativity), and the state (of residence of mother) is code "105" (California), then recode the county (of residence of mother) to the county (of birth occurrence) code.

Evaluation: As in 97-034, use DMR for suspected/pending cases then link to CMR when confirmed. This has worked well in Los Angeles County. In other counties this could be accomplished by first sorting all CMR cards as suspected vs confirmed, then entering only the confirmed cases into the CMR form and the suspected into the DMR form. Only CMRs are transmitted to the state and are reported on the WPR. If a suspected case is confirmed, then the local health department would convert the DMR into a CMR and it would then be transmitted to the State. Problem/Suggestion: We are requesting to have the AVSS Birth Certificate Transfer programs (FSREC & FSHAYES) modified to issue a modem command string to set the hospital's modem to Auto Answer. We have experienced many problems with the modems not connecting.
We have checked the phone lines, modems, and operating system. Our phone lines are clear and without noise. The baud rate on all modems, the communication lines, and the software are all set to 9600. We have saved the Auto Answer condition on the hospital's modem several times. But sometimes the modem will get reset, via the software, and the Auto Answer condition is removed. This is evident when the AA (Auto Answer) light is unlit.
Attached is an example from 5/29/97 communications log with Clovis Hospital. You can see where the system successfully connected and logged onto the laptop (see Line 5). However, the system issued an abort - delineating that the retry had been exceeded. When the system tried to reconnect to Clovis, it was unable to do so (see Line 9). Following some investigation with Kaiser, Clovis Community, and Fresno Community Hospitals we found that these hospitals modems were not set to Auto Answer. At the recommendation of Ellen, we had the users exit AVSS, recycle the mode, and restart AVSS. We issued a successful daytime communication with each of these hospitals. We will be asking the users to re-intialize the modem as part of their daily procedures as a temporary solution.
We believe, that, when the system aborts during a connection the Auto Answer condition is not saved. We are requesting that the system be modified to always issue the Auto Answer command along with any other modem commands. If the Auto Answer condtions exists the issuance of the command will do no harm, and therefore will have no detrimental impact, only a benefieicial one. Orange County also experiences the same problems as Fresno and inclusion of this modification will benefit them as well. We would like to have this change instituted as soon as possible. This procedure (to save the Auto Answer mode) was recommended by Ron Williams during the TAG meeting. However, it has been proven that even though we may issue the save Auto Answer command it is not guaranteed to be preserved, as modem commands are issued from the AVSS Birth Certificate Transfer process which may overwrite the saved settings.

Evaluation: This will work only if the hospital modem is already in the AA state (since it would not answer otherwise). So there is nothing to be gained by issuing such a command upon connection since, by definition, the modem is already in an AA state in order to make a connection. As described in the June 3 TAG meeting, the solution to setting the AA status is in the modem installation procedure: that is, to run INITIALIZE MODEM under SYSTEM MAINTENANCE, then in programmer mode set the modem configuration in non-volatile memory by issuing an AT&W command. This needs to be done only once. One can easily check to see if this command was issued successfully by turning the modem off then back on and checking the status of the AA light( it should be on).

Problem/Suggestion: If a form no. is changed by a Vital records clerk when transferring a record from the Hospital File to the Statistical File, and it does not match the verification form no., no error message appears when the Vital Records clerk "files" the record. We had agreed with John that prompting for the verification of the form no. was not necessary since the record is not printed from the Statistical File. However, the purpose of verifying the form no. is to be sure it is entered correctly. It is possible for the Vital Records clerk to enter the form no. incorrectly. The only time an error message will appear is if both fields are edited in the Statistical File. These two fields should be compared in both the Hospital File and the Statistical File each time just before the record is "filed". If they do not match, the error message should appear.

Problem: Birth and death reallocations are presently executed on an AVSS Project computer. This not only raises questions regarding confidentiality but involves significant telephone tolls.
Solution: The OVR AVSS computer, as managed by Alan Oppenheim and Liz Touhey is the logical vehicle to perform this procedure since it now collects all electronic birth records occurring in California. Thus, reprogram AVSS as follows:
I. FSLCASF2 - Modify routine to create a reallocation global at the OVR level which creates entries sorted by destination LRD codes (both county and city LRDs). (A record being filed could create multiple entries in the reallocation global). This eliminates the need for birth reallocations to city LRD's currently in place at Berkeley and Los Angeles.
II. FSRECOSR - After OVR receives all the LRD transfer records for a given modem, start a second procedure to reallocate records. This entails no change to current OVR transfer software at LRDs, but involves 2 calls to each LRD.
III. FSRLCTRN - Copy existing FSTRNOSR routine and modify to run off the reallocation global.
Incorporate a SENTRLC variable similar to SENTLCA.
Possibly incorporate a Time Delay so that we wait a week or two after receipt of a record before we reallocate to limit errant reallocations.
IV. FSRLCREC - New routine at LRD (Basically the same as FSREC).
Incorporate a SENTRLC variable similar to SENTLCA.
V. Phase in the new procedure on the OSR computer beginning with all births occurring on or after 1/1/98. Continue reallocating deaths on the AVSS Project computer since this approach depends on a complete capture of all electronic records by the OVR computer.
Also, as part of moving the reallocation process to the OVR computer, identify those records to be reallocated to non-AVSS jurisdictions, including non-California residents. Next, print a copy of the birth information on blank VS 10D forms for mailing to each non-AVSS jurisdiction. Sort order to be California LRD, then by other states, and then by foreign countries.

Problem/Suggestion: Problem: Using the existing DMR form in the district, there is no easy way to determine whether or not an individual case has been followed up and completed by nursing. Assuming the implementation of the List to be Sent to Health Department Option for the DMR form, a report utilizing the {SENT} date to describe outstanding cases would be useful.
Proposed solution(s): Develop a simple line listing report which looks for missing dates in the {SENT} field. This report could be entitled "District Outstanding Case Report" or "CD Overdue List for District _________". The report should include the District ID, Name of Case, Disease, Onset and Dx Dates, Age, Sex, Census Tract, and the number of days overdue (Report Date (Today)-Date Received). See attached sample.
It would be helpful to group outstanding cases by census tract, since cases are assigned to Nurses on the basis of census tract. Allow the user to select the age of the cases . That is, ask how old the cases should be in order to get on the overdue list (>30 days, >45 days, <30 days, etc).

2. Enclosed is the format for the 1998 Birth File. The only change I made was to add two characters to every date field so that the century is included as well as the year.
Please review the format to be sure you agree with it. If you find a problem with it, call me. If not, we are asking that you write another Output Tape1 Program to be used on births occurring after 1997. The new program will include the century on all dates.
The old program will have to remain on AVSS until all of 1997 is complete.
As you suggested in our telephone conversation, a check using the infant's birth date will be added to both the old and the new Output Tape1 Programs to be sure the correct one is being used.
You also agreed that you can program this before January 1, 1998. Since I still have programming to do at my end, it would be good if you could finish your work by the beginning of December 1997 so I can complete my programming.

Solution: Display the same information as contained in AMEND DISPLAY at the bottom of the certificate display during AMEND CERTIFICATE.

Problem/Suggestion: Problem: OVR may wish to apply amendments at the state level to LCAs, then have the amended LCA sent automatically to DSB. At present, locally amended LCAs are not sent to DSB, and an LCA is sent only once.
Solution: Whenever an LCA is filed during AMEND CERTIFICATE at OVR, send it to DSB.
Note: this is not the optimal solution since there is a small chance that the LCA could be edited or amended at the local level and negate the OVR amendment before it is sent to DSB. It is therefore recommended that OVR complete the AVSS hierarchical model and execute amendments and other state-specific functions on a SCA (State California) form that would be independent of the LCA.

Problem/Suggestion: Problem: Users may begin using AMEND CERTIFICATE for an LCA, enter the AMEND DATE, then FILE or ^FILE the record. This sets the AMNDAT, but does not mark the record as amended.
Solution: Under these conditions, kill the value for AMNDAT, reflecting the fact that the LCA was not amended.
