AVSS VERSION 4.5 RELEASE NOTES: AUGUST 1995

BIRTH CERTIFICATE, HOSPITAL BIRTH RECORD CHANGES:
94-054In BIRTH CERTIFICATE option, new suboptions GENERATE NEW RECORDER TAPE (1400) and GENERATE NEW RECORDER TAPE TO TERMINAL (1400) have been created based on new CBCspecifications.
94-112ATTENDANT LIST has been added as a suboption under the BIRTH CERTIFICATE option to add, delete, edit and print attendants.
94-127The entry of "N.W." for NOT WEIGHED will not be allowed in field 26-BIRTHWEIGHT.
94-147The following fields will be prompted for and required: 12A-PARENT OR OTHER INFORMANT (signature), 12B-RELATIONSHIP TO CHILD, 12C-DATE SIGNED (parent or other informant), and 13C-DATE SIGNED (attendant or certifier).
94-148When creating a LIST TO BE SENT TO HEALTH DEPT., backing up (^) will be allowed when selecting records.
94-178The entry of values with leading zeroes in fields 20C-FATHER'S EDUCATION, 23C-MOTHER'S EDUCATION, 27A-LIVE BIRTHS NOW LIVING, 27B-LIVE BIRTHS NOW DEAD, 27D-TERMINATIONS BEFORE 20 WEEKS and 27E-TERMINATIONS AFTER 20 WEEKS will be allowed and the leading zeroes will be stripped off. Leading zeroes will also be allowed by counties 01 through 09.
94-181A dash will be allowed in fields 6A-NAME OF FATHER and 9A-NAME OF MOTHER for those cases in which one or both parents have only one name.
95-010HOSPITAL UNSENT LIST OF INCOMPLETES has been added as a suboption under the HOSPITAL BIRTH RECORD option to produce the list of incomplete records not sent to the Health Department.
95-011In the LIST TO BE SENT TO HEALTH DEPARTMENT, the records can be selected and printed in alphabetic order or date-of-birth order. Contact your AVSS programmer
95-012The VERIFICATION LETTER, LIST TO BE SENT TO HEALTH DEPT and NEWSPAPER REPORT can be printed by AVSS with or without the hospital name. Contact your AVSS programmer.
95-013Fields 1A-NAME OF CHILD-FIRST, 1B-MIDDLE, and 1C-LAST (FAMILY) must have acceptable text, a single dash, or be skipped (^S).
95-024The entry and editing of a 9-digit zip code in field 24E will be allowed. The first five digits will be used to check for a valid zip.
95-025The following entries have been added to Race/Ethnic list: 59-FUJI INDIANS, 48-LAHU and 10-YEMENITE.
95-033A social security number will not be issued for a baby when an incomplete name for the child has been entered. The record must have first and last names for SSN. A dash is acceptable for the middle name.
95-045The SENT1 field (indicating the date the electronic record was transmitted from the hospital to the LRD)will be displayed when doing a DISPLAY RECORD.
95-050The century will be added to the LFN effective 1/1/96.
95-051Fields 1A-NAME OF CHILD-FIRST, 1B-MIDDLE, and 1C-LAST (FAMILY) will not allow parentheses.
95-052Field 12B-RELATIONSHIP TO CHILD will not allow the entry of NONE by specifying after the entry of OTHER.
95-054Fields 20B-FATHER'S USUAL KIND OF BUSINESS OR INDUSTRY and 23B-MOTHER'S USUAL KIND OF BUSINESS OR INDUSTRY will not allow the entry of UNEMPLOYED.
95-055Field 24E-ZIP CODE will require an entry of five zeroes if the residence is outside the United States.
95-057Fields 32-FATHER'S SOCIAL SECURITY NO. and 33-MOTHER'S SOCIAL SECURITY NO. will allowan entry of 9 digits, UNKNOWN, WITHHELD, or NONE. An entry of all zeroes within any segment will not be allowed.
95-058The following standard state/country abbreviations have been revised: NE=NEBRASKA (was NB) and NA=NETHERLANDS (was NE). The following country will be removed: NA=NETHERLANDS ANTILLES
95-087Field 12B-RELATIONSHIP TO CHILD will allow the entry of PARAMEDIC.
95-090The short list of OVRS race codes will be displayed before prompting for fields I18-FATHER'S RACE CODE and I21-MOTHER'S RACE CODE.
95-092The HCA-to-LCA linkage/registration has been revised for efficiency. Fields 12B-RELATIONSHIP TO CHILD, 12C-DATE SIGNED (informant), and 13C-DATE SIGNED (attendant or certifier) will not be prompted for if they have been completed by the hospital. Fields I18-FATHER'S RACE CODE, I19-MOTHER'S RACE CODE, I21-FATHER'S SPANISH CODE and I22-MOTHER'S SPANISH CODE will also not be prompted for due to Race and Ethnic Coding enhancements.
95-104The LIST TO BE SENT TO HEALTH DEPT will be redesigned to use barcodes to select records to add to the list.
95-105The Person Selector has been enhanced to use barcodes for record selection throughout AVSS.
95-106LFN ASSIGNMENT has been added as a suboption under the BIRTH CERTIFICATE option to print the LFN and its corresponding barcode using an HP-compatible laser printer.
95-107The validation codes with the validator's initials will be printed on the bottom of birth certificate.
95-108The BPF and its barcode will be printed on the birth certificate in an area that does not interfere with validation codes.
CONFIDENTIAL MORBIDITY REPORT CHANGES:
94-185Three (3) new diseases have been added: ASPERGILLOSIS (ASPER), METHICILLIN-RESISTANT STAPHYLOCOCCUS AUREUS (MRSA), and VANCOMYCIN-RESISTANT ENTEROCOCCI (VRE).
95-037The CMR Weekly Morbidity Report will print the AVSS user's name and phone number as part of the heading.
DEATH RECORD CHANGES:
95-047In the DEATH RECORD option, a new REGISTER CERTIFICATE has been developed. Included in the new registration will be twenty (20) required fields and thirty (30) optional fields. Thirty-seven (37) fields which were included previously will no longer appear as prompts.
95-098Field 119-MANNER OF DEATH codes have been revised as follows: 1=NATURAL, 2=ACCIDENT, 3=SUICIDE, 4=HOMICIDE, 5=PENDING INVESTIG, 6=COULD NOT BE DET, and 9=UNKNOWN.
REPORT GENERATOR CHANGES:
95-018A new standard report, #CTRAN1L, has been added to produce a REGISTRAR TO RECORDER LIST.
95-088A new standard report, #FLATR2T, has been added to produce the list LATE SUBMISSION BY FACILITY NUMBER for all facilities. The standard report, #FLATR1T, has been modified to automatically list all facilities by hospital code number.
